To make filling out the promissory note an easier task - print out the following instructions:

• Fill in your name
• Fill in your student number
• Fill in your complete address - city - province - postal code
• Fill in your home phone number
• Fill in phone number to contact you during the school year
• Fill in your date of birth
• For type of deferment - circle OSAP
• For method of payment - fill in your credit card information for payment OR attach a cheque for $40 (your application cannot be processed without this payment)
• Calculate your total amount due (the amount on your invoice plus a $40 deferment fee) OR after registration is complete the total amount due is (the total account balance from the account summary by term screen plus a $40 deferment fee)
• Fill in the total amount due behind the words, "I promise to pay the total sum of $"
• Be sure to read the entire promissory note - this is a legal document
• Have someone witness your signing of the promissory note
• Sign the promissory note (mandatory to get a program seat)
• Date the promissory note (mandatory to get a program seat)
• Have witness print their name and phone number and then sign the promissory note
• Include a signed photocopy of a PHOTO ID (eg. drivers license) (mandatory to get a program seat)
• Remember to complete a second copy or take a photocopy of the completed promissory note for your own records.

A fully completed signed and witnessed promissory note and signed photocopy of a PHOTO ID with deposit payment - can be mailed or delivered to the finance department. Operating hours are from 8:30 a.m. to 4:30 p.m., Monday to Friday.
